Dear Valued Client,

Now is the time to start preparing for your 2019 individual income tax return. Your team at Restivo Monacelli is committed to truly understanding your tax matters in order to maximize your tax benefits and minimize your tax liabilities. Below is a link to our 2019 individual income tax questionnaire, which is designed to aid us in the preparation of your income tax return. In addition to the questionnaire, a charitable contribution deduction worksheet has been included. We are requesting that you fill out and return the questionnaire and charitable deduction worksheet along with your income tax documents.

When putting together your income tax documents, please be aware that the Internal Revenue Service matches information returns/forms with amounts reported on tax returns. Accordingly, it is important that you provide us with all of the income tax documents you receive. The following are some of the more commonly issued income tax forms and schedules that are received by individuals that would be required for the preparation of your income tax return:

  • W-2 (Wages)

  • Schedules K-1

  • 1099-R (Retirement) / 1099-INT (Interest) / 1099-DIV (Dividends) / 1099-B (Brokerage Sales) /1099-MISC (Rents, etc.) / 1099 (any other)

  • 1098-Mortgage Interest / 1098-T (Education)

  • 1095-A (Health Insurance Market Place Statement)

  • 1095-B (Health Coverage Statement)

  • 1095-C (Employer-Provided Health Insurance Coverage Information)

To continue providing quality services on a timely basis, we urge you to collect your information as soon as possible. If Schedule K-1 information from “passthrough” entities such as partnerships, trusts and S corporations is the only data you are missing, please send the data you have assembled and forward the missing information as soon as it is available.

The filing deadline for your 2019 individual income tax return is Wednesday April 15, 2020. In order to meet this filing deadline your completed income tax questionnaire and original tax documents need to be received no later than Monday, March 16, 2020. Any information received after that date may require that an extension of time be filed for your income tax returns. If an extension of time is required, any tax due must be paid with that extension. Any taxes not paid by the filing deadline may be subject to late payment penalties and interest.

We look forward to maintaining and growing our relationships with you. If you have any questions, please do not hesitate to contact us at 401-273-7600.
